About You

Bega Garnbirringu is seeking a dedicated and compassionate Aboriginal Health Practitioner to join our friendly and supportive Clinic team.

The Aboriginal Health Practitioner will provide comprehensive, culturally safe and holistic primary health care to Aboriginal and Torres Strait Islander clients.

The position will provide quality health care services including health assessment, treatment, case management, health education, care coordination and follow-up care.

The successful applicant will work collaboratively with clients, families, community members and other health professionals to support improved health outcomes.

Key Responsibilities

Provide culturally safe, high-quality primary health care to Aboriginal and Torres Strait Islander clients.
Conduct health assessments, clinical assessments and appropriate screening.
Provide treatment, health education, health promotion and preventative health care.
Develop and maintain appropriate care plans and provide case management and follow-up.
Work collaboratively with medical, nursing, allied health and other service providers.
Maintain accurate, confidential and timely clinical records in accordance with organisational policies and procedures.
Support continuity of care and appropriate referrals to internal and external health services.
Promote culturally appropriate health practices and contribute to improving health outcomes for Aboriginal people.
Participate in outreach and community-based activities as required.
Essential Criteria

Applicants must have:

Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care (Practice).
Current AHPRA registration as an Aboriginal Health Practitioner.
Strong communication and interpersonal skills, with the ability to work effectively with Aboriginal people, families and communities.
Sound understanding of the health issues and challenges impacting Aboriginal and Torres Strait Islander people.
Demonstrated ability to work effectively within a multidisciplinary team, including good conflict management skills.
Good computer, written communication and organisational skills.
Current Working with Children Check (WA), National Police Clearance issued within the last three months, and a current WA C Class Driver's Licence.

Desirable Criteria

Experience working in Community Health or Aboriginal Community Controlled Health Services.
Current Immunisation Certificate.
Current Phlebotomy competency.
Experience using Communicare or a similar clinical information system.
Experience providing outreach or community-based health services.
Willingness to undertake outreach travel, including overnight stays of up to four nights when required.
