Your new role
As a Senior Assessment and Facilitation Officer you will contribute to the development, delivery, performance and quality of the Home and Community Care Program for Younger People (HACC PYP), providing supervision, consultation and support to staff within the HACC PYP team.

You will complete holistic assessments of need to support the development of goal directed support plans promoting a strengths-based approach in partnership with service providers to ensure that individual client needs can be met.

Your skills and experience
You will meet the person specification in the position description, including having a Tertiary qualification in Social Sciences, Disability Studies or Allied Health professions, or equivalent qualifications with demonstrated industry and leadership experience.

You will also have:

Current driver’s licence valid in Victoria
Current First Aid and CPR Certificate Level 2
Demonstrated ability to provide safe environments for children and young people and protect them from abuse and neglect.
Knowledge of principles of community inclusion and participation, with experience in delivering a response or project as a result to a community need or service gap.
