Improving the equity of the health outcomes particularly for Pacific, Māori, and low socioeconomic groups is a priority for ProCare, who is responsible for the largest Māori and Pacific population in Auckland. The recent Health and Disability Review highlighted the need for more services to have an outreach element making it easier for whānau to stay connected to the health system. Home-based support is seen as a model that will improve health equity based on patient needs.

The RN plays a key role in effectively implementing our outreach mobile service that aims to improve the health outcomes of ProCare enrolled families with the highest unmet health needs. The role will work alongside the Project Lead to work with the practices and community to deliver clinical care, and is supported by our Equity team and multi-disciplinary Squad.

Key skills and experience registered Nurse 5 years post qualification experience, preferably in a primary healthcare setting or general practice. Current annual practicing certificate (NZNC) and ideally immunizations/ cervical certified . A full, clean driver's license (work car and/or mileage provided). Broad understanding of Pacific cultures and.

Confident communicator a proactive approach to daily mahi, self-directed, and ability to work autonomously and collaboratively. A desire to understand and appreciate Te Tiriti o Waitangi and its application to ProCare and how it shapes our efforts. Whānau focused with a passion for playing a part in improving population and whānau health across Tāmaki Makaurau.
