Your opportunity

As a Community Reablement Lead, you'll drive and embed reablement practice across ECH, shaping how services are designed, delivered and continuously improved. Working across multidisciplinary teams, you'll provide clinical leadership, support complex care decisions, and build workforce capability in strengths-based, person-directed care.

Alongside managing a flexible caseload of clients with complex needs, you'll partner with leaders to implement service improvements, support aged care reform and Support at Home changes, and champion innovative approaches that help older people maintain independence, wellbeing and quality of life at home.

You’ll make an impact by

Leading and embedding clinical care management and reablement practices that drive sustainable improvement.
Providing expert clinical guidance and oversight for clients with complex or changing needs.
Driving practice and service improvements to enhance client outcomes and service effectiveness.
Building workforce capability through coaching, mentoring and evidence-based practice support.
Partnering with leaders and teams to implement reforms, improve service delivery, and promote person-directed, reablement-focused care.
What you’ll bring

To succeed in this role, you’ll need the following qualifications:

Tertiary qualification (Bachelor's degree or higher) in Allied Health.
Current and unrestricted registration with AHPRA or other relevant professional body.
Minimum five years' post-registration clinical experience in aged care, community health, rehabilitation, hospital transition care, palliative care or a related setting.
Demonstrated experience in care management, case coordination or clinical service delivery.
Current driver's licence.
National Police Check.
We would also love you to bring:

Strong understanding of contemporary reablement, rehabilitation or wellness approaches, and the ability to translate into practice.
Proven ability to lead practice improvement, influence multidisciplinary teams and build capability in complex service environments.
Exceptional communication, negotiation and influencing skills.
Strong clinical reasoning and risk management capability.
Ability to analyse service trends and use data to inform decision-making and quality improvement.
