Are you passionate about supporting people following a suicide attempt or suicidal crisis to build safety, connection and recovery?

Anglicare Tasmania is seeking a permanent, part-time (53.2 hours fortnightly) Practitioner – Health and Wellbeing (Universal Aftercare Program), based in Launceston.

About the role:

The Practitioner - Health and Wellbeing (Universal Aftercare Program) is responsible for providing skilled, recovery-focused psychosocial support to people aged 16 years and over following a recent suicide attempt or suicidal crisis.

The Universal Aftercare Program is a non-clinical suicide aftercare service that supports people during a period of heightened vulnerability following a suicidal crisis. Practitioners work collaboratively with individuals, their chosen supports and relevant health and community services to strengthen safety, wellbeing, connection and recovery.

Working within a clearly defined non-clinical scope, the role combines psychosocial assessment, engagement, case coordination, collaborative safety planning and practical support. Practitioners provide flexible outreach, assist people to navigate challenges impacting their wellbeing, and help connect individuals with the services and supports they need to move forward. This includes working alongside people experiencing a range of psychosocial challenges related to housing, relationships, employment, education, financial stress and social isolation.

The role requires strong interpersonal skills, sound judgement, and the ability to work effectively with people experiencing distress, complex circumstances or barriers to engagement. Practitioners play a key role in promoting recovery, strengthening protective factors, and supporting individuals to build safer and more connected futures.

To be successful you will have:
The ability to provide support to people accessing Anglicare’s Community Services within the Anglicare practice framework and other standards related to the Community Services sector.

Demonstrated case management skills and ability to work effectively with clients, family members and service providers to identify solutions.
An ability to recognise and understand people’s intrinsic resourcefulness.
Sound understanding of the personal, family, and social issues associated with suicidal ideation and attempts, including awareness of groups identified to be at increased risk of suicide.
Good understanding of recovery principles and/or strengths-based principles.
Provide support in a manner consistent with Anglicare’s values and practice principles.
Excellent communication skills, particularly with individuals and their families/carers who may be experiencing significant distress.
High level of competency using technology to coordinate client care, to collaborate with other providers and report outcomes.
Experience liaising with external service providers including specialist mental health services, GPs and other community-based services and organisations.
Demonstrate high level of communication and interpersonal skills with people who access our services, colleagues, other service providers and stakeholders.
Undertake outreach activities in accordance with service delivery requirements to ensure effective engagement with clients and stakeholders.
