Your new role
As a Carer Wellbeing & Support Planner for Carer Gateway, you’ll work alongside unpaid carers to understand their circumstances, priorities and the impact of their caring role. Using the Carers Star™ tool, you’ll complete assessments and develop practical, goal-directed support plans.
Your work will include:
Providing carers with information, guidance and individual support.
Arranging brokered services within approved funding limits.
Engaging with carers through coaching, peer support and community events.
Helping carers plan for future needs and periods of difficulty through referrals to other relevant supports.
About the program
Unpaid carers play an essential role in supporting family members or friends who are ageing, living with disability, experiencing mental health challenges or managing other health and wellbeing needs. While caring can be rewarding, it can also affect a person’s health, relationships, work and ability to plan for their own future.
Our Carer Wellbeing team works with carers through the Carer Gateway program to understand their individual circumstances and connect them with timely, practical and person-centred support.

Your skills and experience
You’ll bring a strengths-based and compassionate approach, with the assessment skills and service knowledge to help carers identify their goals and access the right support.
You will also bring:
A tertiary qualification in disability, social work, health, psychology, community development, community services or a related discipline.
An understanding of the experiences and challenges associated with an unpaid caring role.
Strong engagement, needs-assessment and care-planning skills.
Knowledge of community services, supports and referral pathways relevant to carers and the people they support.
