Communify is seeking passionate and experienced Mental Health Practitioners to join an exciting new Mental Health Care Partners (MHCP) Pilot at the Redcliffe Medicare Mental Health Centre.

This innovative Brisbane North PHN initiative brings together multidisciplinary mental health professionals and General Practices to provide coordinated, recovery-focused care for people experiencing complex mental health needs.

Our Medicare Mental Health Centres provide free, welcoming, multidisciplinary support for adults experiencing mental health concerns through assessment, care coordination, peer support, psychological therapies and recovery-focused interventions.

About the Role

As a Mental Health Practitioner, you'll work as part of a multidisciplinary team delivering evidence-based, recovery-oriented mental health care to people with complex needs.

Working closely with participants, General Practitioners, peer workers and community services, you'll help people navigate the mental health system while supporting them to achieve their recovery goals.

Your responsibilities will include:

Conducting comprehensive biopsychosocial assessments.
Developing collaborative recovery and shared care plans.
Delivering therapeutic and psychosocial interventions.
Providing community outreach and in-reach support within participating GP practices.
Coordinating multidisciplinary care and facilitating warm referrals.
Supporting participants to access the right services at the right time.
Maintaining high-quality clinical documentation and participant records.
About You

You are a compassionate, adaptable and collaborative mental health professional who enjoys working within multidisciplinary teams and building strong relationships with participants and community partners.

You'll bring:

A qualification in Social Work, Psychology, Occupational Therapy, Mental Health Nursing, Behavioural Science, Counselling, Human Services, Community Services or a related discipline.
Experience supporting people experiencing severe and complex mental health concerns.
Strong knowledge of trauma-informed, recovery-oriented and strengths-based practice.
Experience undertaking assessments, care coordination and psychosocial interventions.
Excellent communication and relationship-building skills.
Confidence working collaboratively with General Practice, health services and community organisations.
Strong organisational skills and experience using electronic client management systems.

Essential Requirements

Current Queensland Driver Licence
Current Blue Card (or ability to obtain)
NDIS Worker Screening Clearance (or ability to obtain)
Current First Aid & CPR (or willingness to obtain)
National Police Check
