What You’ll Do:
In this dynamic and rewarding role, you’ll:
Partner with participants to design and implement personalised, strengths-based NDIS plans.
Connect people with the services and supports that align with their goals and needs.
Use recovery-oriented and person-centred approaches to build skills and confidence.
Coordinate service delivery, monitor progress, and adapt plans as required.
Collaborate with a multi-disciplinary team and contribute to service innovation.
Maintain accurate records and ensure alignment with NDIS practice standards.

What You’ll Bring:
Significant experience in the disability or human services sector.
Knowledge of intervention models and ability to coordinate services.
Specialist knowledge of NDIS and related support services.
Strong problem-solving and conflict-resolution skills.
Ability to advocate effectively for participants and promote their independence.
Your Toolkit:
Tertiary qualification in community services, social work or equivalent and extensive community work and training (essential)
Current DHS Working with Children Check (essential)
Current DHS NDIS Worker Screening Check (essential)
Current SA driver’s licence (essential)
Current Safe Environments for Children and Young People – Through Their Eyes Certificate (essential)
NDIS Orientation Module (essential)
