Social Worker – Full-Time
Location: Aegis Banksia Park Transitional Care Program (TCP), Calista
About the Role
Join our dedicated team at Aegis Banksia Park TCP, where we empower older adults to optimise their functioning and independence and facilitate their transition from hospital care to home or permanent aged care. Our goal-oriented, time-limited, therapy-focused program supports older people who have completed their acute or subacute hospital care, providing a seamless continuum of care to enhance their quality of life prior to their next care environment.
We are seeking a compassionate and skilled Social Worker to join us on a permanent full-time basis. In this key role, you will work closely with residents, families, and multidisciplinary team members to deliver professional social work services tailored to the unique needs of each individual.
Key Responsibilities:
Provide direct, professional social work services to residents and their families.
Coordinate and support admissions through engagement with Residents and Families to establish and meet care goals.
Coordinate discharges through ongoing liaison with Residents’ families and prospective Aged Care providers or Home Care services within allocated timeframes.
Maintaining clear communication with Department of Health TCP Hub on an ongoing basis.
Actively participate as a key member of the multidisciplinary team, including in multidisciplinary meetings and resident case conferences.
Liaise with residential aged care facilities and community service agencies to facilitate continuity of care.
Maintain up-to-date knowledge of community-based services and resources.

What We’re Looking For:
You must hold a minimum of a four-year Bachelor of Social Work (BSW) or a two-year Master of Social Work (MSW), both of which must be accredited by the AASW.
Proven ability to work effectively within multidisciplinary teams.
Strong verbal and written communication skills.
Excellent interpersonal and relationship-building skills.
Knowledge of the Guardianship and Administration Act 1990 and State Administration Tribunal process.
Experience conducting psychosocial assessments, interventions, goal setting, and care planning.
