Health Improvement Practitioner Te Awakairangi Health Network location Wellington. $80,000 – $90,000 per year.
We are looking for a registered health professional who is passionate about people and health, to take up a dynamic position within the Access and Choice team. The role will be based in the Hutt Valley area and the successful candidate will have enthusiasm and drive to work in this space with confidence.
About us te Awakairangi Health Network is a primary care network focussed on achieving equity and improving the quality of our health services for whānau living in the Hutt Valley. We work with general practices and community service providers to promote and empower people to receive the healthcare services they need and deserve.

Key features of the HIP role include generalist- HIPs can work with all registered whairoa for any problem to improve physical and psychological wellbeing. Accessible- A free service where people can get the help they need at the time they need it, and in most cases, on the same day.
Team-based- Part of the general practice team and working flexibility to support the team and whairoa. This includes individual consults, group work facilitation and supporting practice colleagues with education and advice.
High productivity- Providing brief interventions with a mix of same day and pre- bookable appointments available. Educator- Offering education to whaiora and the general practice team. Routine pathways- Developing and supporting routine pathways for high needs populations.
