Care & Connects is an innovative NDIS provider who is dedicated to delivering the best quality support to each of our participants. This high performing team works across our Supported Independent Living (SIL) homes and supports people who experience significant and challenging behaviors. We are looking for team members in various suburbs in the northern and southern areas of Adelaide, with a preference for Para Hills, Parafield Gardens and Salisbury

About you, you are adaptable, resilient, and prioritize the needs of participants. With a minimum qualification of Certificate 3 in Individual Support, you have experience assisting individuals with exceptional behavioral needs. Your role involves assisting with daily activities in supported accommodation, such as meals, personal care, domestic tasks, and community engagement. This hands-on position requires active individuals who are dedicated to building genuine relationships with both participants and co-workers.
Qualifications minimum Certificate III / IV in Disability / Mental Health or equivalent. Medication Administration Accreditation. NDIS Worker Screening. Manual Handling Training. First Aid and CPR. CPI/MAPA/Mental Health First Aid is desirable. Any additional trainings such as PEG/Bowel/Diabetes is an advantage. 100 Points ID. AU Driver's licence.
Schedule Day shift- 7 am - 3 pm. Afternoon shift - 3 pm - 11 pm. Active and Passive night shifts are typically combined with either noon or morning shifts. Rotating Weekends. Short Shifts. Shifts rotate 24 hours a day, 7 days a week, based on business requirements. Kindly apply only if willing to work on the above-mentioned shifts.
Your tasks may include but not be limited to the following providing personalized care and support to our participants, including personal hygiene; meal preparation; medication; and activities of daily living. Implementing positive behaviour support plans and crisis intervention strategies.
Provide emotional and practical support to participants according to their individual needs. Monitor participant’s progress and report any changes or concerns as required. Maintain accurate records and documentation in line with NDIS and organisational requirements.
