Calling Registered Nurses
Weekday shifts - between M-F (4 days a week)
Working from home role
This role is part of a team that will provide care coordination, care navigation, case management and health coaching to a cohort of clients living with a chronic illness and or complex needs resulting in multiple admissions to hospital. This primarily community-based and working from home role will act as the chain linking the multiple parts of the health system from a client’s perspective and will be a consistent contact point along their journey. The role will also involve a component of direct care clinical assessment and intervention aimed at preventing unplanned or inappropriate after hours admission to hospital.
4 days across Monday to Friday 08:30am - 4:36pm (No Weekends or Public Holidays)
The Opportunity
Silverchain are seeking an experienced Registered Nurse who will be responsible for care coordination, navigation and health coaching services via face-to-face or telehealth to people that have a chronic condition and complex needs in South East Melbourne. This involves developing an individualised care plan, referring to other providers as needed, navigation across multi-disciplinary services required to fulfil this care plan, service linkage such as transport, and supporting self-management principles.
About You
Relevant qualification in Nursing with demonstrated experience in field of expertise
Experience in delivering chronic condition management, health care navigation, health coaching and the principles of health literacy
Well-developed communication (verbal, written and listening) and interpersonal skills to liaise with clients, carers, families, and other health care professionals
Demonstrated ability to provide education, support and coaching to clients and their carers
Demonstrated time management and organisational skills to effectively plan and prioritise work
Demonstrated ability to work both autonomously and with a team to participate and contribute to program and organisational objectives
A demonstrated empathetic and flexible approach in all interactions with clients, carers and other health providers
Ability to work autonomously or with minimal direct supervision
Based on Silverchain Group Policy and Government Directions, you will be required to provide evidence of vaccination against COVID-19 including booster, or evidence of a medical exemption in order to commence employment with Silver Chain.
