We are seeking an experienced and adaptable Care Manager to provide senior clinical and operational support at Wesley House Residential Aged Care, located in Semaphore Park.
This position forms part of a rotating Care Manager model, where three senior leaders work together to ensure year-round leadership and oversight across weekdays and weekends. This structure provides crucial clinical governance and continuity during weekends, when support is often most needed.
Reporting to the Residential Operations Manager, this key leadership role ensures consistent, high-quality care and strong clinical governance, wherever it’s needed most.
You will support site teams to meet Aged Care Quality Standards and guide staff in delivering best-practice, person-centred care.
Roster Details:
This role operates on a rotating roster, providing both flexibility and variety throughout the year:
For 8 months of the year, you will work Monday to Friday
For the remaining 4 months, you will work Thursday to Monday, inclusive of weekends
This structure supports continuity of care across the service and enhances leadership coverage during weekends and critical periods.
In this role, you will:
Provide Clinical and Operational Leadership: Step into leadership roles at any of our five residential aged care homes to support during periods of leave, transition, or change.
Drive Quality and Compliance: Ensure adherence to Aged Care Quality Standards, AN-ACC funding requirements, and accreditation processes.
Mentor and Coach Teams: Build staff capability through supervision, reflective practice, and on-site mentoring.
Respond to Clinical Complexity: Support the assessment and care of residents with complex needs, ensuring person-centred, holistic care planning.
Ensure Care Continuity: Maintain clinical oversight and continuity of services, especially during transitional or challenging periods.
Lead Improvement Initiatives: Conduct audits, identify risks, and support implementation of quality improvement plans.
Participate in On-Call Roster: Provide after-hours clinical support across the aged care portfolio on a rotating basis.
To be successful in this role, you will have:
Current, unrestricted registration as a Division 1 Registered Nurse with AHPRA
Minimum 5 years' post-registration experience in aged care, including residential operations
Proven leadership capability and advanced clinical problem-solving skills
In-depth knowledge of Aged Care Quality Standards, AN-ACC, care planning, and documentation
Excellent communication and interpersonal skills with the ability to lead multi-disciplinary teams
Proficiency with clinical systems and electronic documentation
Current SA Driver’s Licence and NDIS Worker Screening Check (essential)
Postgraduate qualifications in Aged Care, Leadership, or Health Management (desirable)
