Health Improvement Practitioner (HIP) TE HIKU HAUORA LTD location Northland.
About the role as a Health Improvement Practitioner (HIP) at Te Hiku Hauora Ltd, you will play a crucial role in providing evidence-based, person-centred psychological interventions to individuals and whānau (families) in the Kaitaia Northland region. This full-time position is an exciting opportunity to make a meaningful difference in the lives of the local community.
What you'll be doing deliver brief psychological interventions and support the development of self-management strategies for clients experiencing mild to moderate mental health and addiction issues. Collaborate with primary care teams, social services, and community organisations to ensure seamless, wraparound care for clients.
Provide assessment, formulation, and treatment planning to enable client-directed goal setting and achieve positive health outcomes. Maintain comprehensive, accurate, and timely client documentation in line with organisational policies and professional standards. Participate in regular clinical supervision and contribute to the continuous improvement of the HIP service.
What we're looking for we will be looking for a Registered Nurse or a Registered Social Worker with community mental health experience, with the view of the successful person being a key member within our holistic General Practices. There will be training available and access to support for the role. And the person will be a key part of our emerging Mental Health Hub.
A relevant tertiary qualification strong clinical skills in assessment, formulation, and treatment planning, with a person-centred, strengths-based approach. Excellent communication and interpersonal skills, with the ability to build rapport and work collaboratively with clients, whānau, and multidisciplinary teams.
A commitment to cultural responsiveness and working effectively with diverse communities. Registration with the appropriate professional body (e.g. Nurses Council) or eligibility for registration. Experience working within a kaupapa Māori environment.
